                     The School clinic is available to assist students and staff with sudden onset of illness, emergencies, medication administration, Health education, safety, wellness management of chronic health conditions and any preparation necessary to have the most successful and productive school day possible. I strive to maintain open communication with all families that we serve in order to meet the needs of your students.
When Health information needs to be shared I will disseminate the information as quickly as possible.
Families, PLEASE keep your emergency cards updated. Family contacts can be made more efficiently with current numbers on file. THANK YOU !


Immunization reminder:
                   All rising 6Th graders (and rising 7Th and 8Th graders also) must have a TDAP booster for Middle School entrance! Parents should consult with their physician or the PWC Health Department as their student turns eleven and have this booster as soon as possible. PROOF of immunization must be forwarded to the school. Students without proof of this important vaccine cannot attend middle school-6Th graders may be excluded from school in September 2011.

Concussion Information
Please see LAKE RIDGE MIDDLE SCHOOL home page for CONCUSSION training. All students who wish to try out for any school sport must have attended the training with a parent or guardian and had a sports physical also to try out. Concussion guidelines in general will be followed for all activities. Thank you for your attention to this VERY IMPORTANT information.
NEW HEALTH PLANS
Parent's and Guardians~
              Please prepare new health plans,asthma and allergy action plans for the upcoming 2011-2012 school year. All necessary forms are here on the LRMS clinic website.Thank you for your efforts.
REASONS FOR WHICH A CHILD MAY BE SENT HOME OR EXCLUDED FROM SCHOOL
As per PWCS regulation 758-1
1. Fever of 100 degrees  and over -exclude student for at least 24 hours.
2.Conjunctivitis(pink eye), strep infections,ringworms,impetigo or all infections that must be treated with medication for a min. of 24 hours.
3. rash of unknown origin.
4. Head injury
5.Severe coughing or difficulty breathing
6.Excessive cold symptoms
7.Diarrehea or vomiting- exclude until symptom free for 24 hours.
8.Stiff neck associated with fever or recent injury.
9. Inadequate immunizations.
10. Refer to the Virginia Dept. of Health Communicable Disease reference Chart for additional information.
Clinic Information
MEDICATIONS~

Please be aware that all prescription and over the counter medications require written consent from Physicians and parents. The medication form is available on the PWC web site, at school, and will be available here on the LRMS web site.
The only medications that any student may carry WITH CONSENT are Inhalers or epi-pens for life threatening medical conditions.
All Inhalers for students who are diagnosed with Asthma must be accompanied with a Physicians Asthma action plan which must be kept on file with the School Nurse. The plan is to be renewed each school year.

Allergies~ All students with life Threatening allergies must have a Physician signed Allergy action plan on file with the School Nurse. All food and environmental allergies will be documented and shared appropriately
 with school staff that will be in contact with that student for general safety and wellness. This medical plan also must be renewed each school year.

General Information~

In compliance with the Strategic plan of Lake Ridge Middle School, it is my goal to meet the needs of students and staff on a daily basis and communicate with families in the most timely manner possible the needs and concerns of their student or family member. It is the goal of student health services to assist students to have the most productive school day possible.
